You’ll need to submit your nomination within 60 days of the trip. If it’s not received and processed within this timeframe, it won’t be accepted.
To avoid delays, we recommend submitting your nomination as early as possible—ideally within 30 days of travel—to allow time for postage and processing.
- Download and complete a statutory declaration form. You will need to provide:
- the other driver’s name and address; and
- this must be witnessed and signed by an 'authorised person' (eg: a legal practitioner or a Justice of the Peace).
- Complete a separate statutory declaration for each toll notice.
